Documents required for your 408 visa application –

1. Scan of your passport your photo, personal details, and passport issue and expiry dates.
2. Any other documents that prove your identity card you have such as national identity cards or drivers licenses. Change of name proof if applicable.
3. A letter or contract, from your workplace confirming job or job offer, showing their ABN and address and a contact name, phone number and e-mail address for that person.
4. Health Insurance documents.
5. Brief summary of your work duties – a couple of sentences. 
6. Addresses and dates (dd/mm/yy format) for any locations you have lived outside Australia in the last 10 years.
7. Place of birth, state/province, area code. 
8. Current address, e-mail address and phone number.
9. Copy of your current visa. 
10. We will e-mail you a form 956 appointing us to represent you as well as an agreement – once these are signed we can launch your application.
